need immediate medical attention, please call 9-1-1 or go to the nearest Emergency Room.  

                           Promedica Clinic, PC prefers to schedule  
                                                   appointments, however, every effort    will be made to accommodate non-acute 
walk-ins and/or same day appointments.  

 Please bring a list of all medications with you
 at the time of your medical appointment. 

Please make every attempt to keep your scheduled appointments. In the case of cancellation, please call 24 hours in advance if possible. 
If you have been injured in an accident, it may be possible to assign insurance benefits to assist with payment of patient balances. The following information is necessary for injury patients: insurance carrier, claim number, date of injury, adjuster contact information and attorney information if represented. 

If you are injured on the job, approval by your employer and/or work comp insurance company is necessary before services may be provided. Please call (901) 526-7695 for further information.
